This is a pilonidal sinus with multiple sinus tracks on either side of natal cleft. There is evidence supporting both a congential and acquired cause. The disease is complicated by acute infection and may present as a pilonidal abscess. The standard surgical management of the chronic condition is wide excision using various techniques to primary close the defect. Bascon's cleft closure technique is currently popular with some reports of excellent results but other reports suggesting no advantage over conventional surgery.
