The mechanism of fracture is usually forced inversion causing an avulsion of the base of the fifth metatarsal. Although the patient will present with a sprained ankle the point of maximal tenderness will be over the base of the fifth metatarsal. On Xray the fragment is usually only slightly displaced. If it is grossly separated, non-union can develop with associated chronic pain. This can be treated with internal fixation.
