This is gall stone ileus,
a large pigmented stone having impacted in the mid ileum. It presents as small
bowel obstruction, the stone having fistulated from the gall bladder into
the duodenum. Plain xray may show small bowel obstruction with gas in the
biliary tree. The stone itself is usually not radio opaque and is therefore
not visible on plain xray.
