This is an acute paronychia
of the index finger with oedema of the finger and cellulitis. Pus can be seen
deep to the finger nail and at the tip of the finger. It requires surgical
drainage by removal of the finger nail and appropriate antibiotics eg penicillin
and flucloxacillin.There may be long term, even permanent stiffness of this
important finger about which the patient should be warned.
