There is a superficial abscess at the base of the thumb with surrounding cellulitis and lymphangitis extending along the forearm towards the elbow. Streptococcus and Staphylococcus are the most likely organisms and the patient should receive intravenous penicillin and flucloxacillin in the first instance.
