This is a case of Fournier's gangrene which is necrotising fasciitis involving the perineal area. Organisms can be anaerobic eg bacteroides or gram negative aerobic bacillior enterococci. There is rapid destruction of subcutaneous tissues and septicaemic shock. Treatment consists of wide excision of all involved skin and subcutaneous fat with high dose broad spectrum antibiotics. Outcome is often fatal.
